Offensive-Defensive Mental Skill Survey (copyrighted 2011)
If you want to help your players determine their offensive and defensive mental skills strengths and weaknesses, download this survey. It will reveal which of the defensive mental skills are strengths (refocus, confidence, intensity control) and which of the offensive mental skills are strengths (ability to stay focused, preparation, physical preparation, quality trainer). This survey is 28 items and includes a scoring key. Scores range from 0 (never) to 6 (always )
Team Leadership Survey (copyrighted 2011)
This survey has been used by top NCAA Division I teams and national-level programs in numerous countries (New Zealand, Scotland, Australia) to determine their team captains and to help determine who is doing what in terms of team leadership practices. The ideal is to have each team member complete this survey by writing the name of one to two teammates who best exemplify the particular leadership quality or leadership action. Then, based upon the patterns of the responses (number of times certain athletes were cited), they are to write the name of one to two teammates who would make our ideal captain. This way, it is far from being a popularity contest, and the feedback can be used to assist these captains in leading this team, since this came from the teammates (followership). This survey helps to determine the Practice Leader, Supportive Leader, Go-To leader, Challenger, Team Builder, and Leadership Qualities.
This survey includes 30 items and includes a SUMMARY SHEET. I usually add up the number of times each athletes’ name was listed to get a frequency rate.
Leadership Skill-Set Assessment (copyrighted 2011)
This 60 item survey was constructed from coaches and past captains that Dr. V has been so fortunate to collaborate with. This can be used for those who were named captain or if you would like to do more with individual leadership with your team, have them all complete it. I use this feedback to help assign leaders with particular responsibilities based on strengths and team needs. This will also provide a lot of valuable feedback for leadership meetings. This survey is scored from a 0 (never) to 6 (always).

Quality of Practice Survey (copyrighted 2011)
This is a 32-item survey which looks at an often overlooked issue = the quality of an athlete’s training session. This survey is designed to reflect my model of QUALITY PRACTICE which encompasses the quality of one’s attitude for practice; what does the athlete do to prepare for practice; do they use any execution routines to maintain quality effort and execution; and finally, once practice is completed, do they reflect on what worked, what didn’t work in that practice session and what will they do in tomorrow’s practice? Scores range from a 0 (never) to 5 (always).
Capacity for Excellence Questions (copyrighted 2011)
This questionnaire consists of open-ended questions which represent my applied consulting model, which works to improve upon the 9 aspects to a performer’s performance potential. These questions are designed to provide important information that should lead to dialogue between athlete and coach and part of the action plan.
The 9 areas include: Physical Toughness, Technical Toughness, Tactical Sense/Application, Team Play, Quality of Practice, Leadership, Mental Toughness, Emotional Toughness, & Inspirational Toughness.
